Registration Form - Drumming For Jared Program

All new students are required to register to secure private lessons with the Drumming For Jared program. This registration process helps to reserve the student's requested lesson day and time once the registration process is complete. Program slots are based on availability. Social service agency verification is also a required step for enrollment into this program.

NOTE: Registration is for new students only and must take place at least three days prior to starting your lesson program. Registration must also occur no more than two weeks prior to the start of the program. We accept students of all ages. A parent or legal guardian must fill out the registration form if the student is under the age of 18.

* Next, register using the form below.
* Next, download and sign the "Release of Information" form (if applicable) and submit it to your social service agency so that we may verify your low-income status.
